About Your First Office Visit

Dr. Ron Banik and his team always look forward to meeting new patients! In addition to gathering your medical history, we will perform our comprehensive new patient exam. We use state-of-the-art technologies including ultra-low radiation digital x-rays and thorough clinical techniques where you can expect the following:

Biomechanical Exam

looking for tooth decay, broken teeth, defective fillings, evaluating risk for future problems, etc.

Periodontal Exam

Measuring gum health, evaluating risks for gingivitis and periodontal (gum AND bone) disease. NOTE: If gum health is normal, a professional cleaning MAY be able to be performed at this visit

Oral Cancer Exam

Evaluating any abnormalities and risks

Bite and TMJ Exam

looking for pain or damage to teeth due to an incorrect bite and/or possible jaw problems

Dentofacial Exam

Evaluation of tooth alignment and position in relation to the face, lips, etc

Dental Cleanings

For children 16 and under, a professional cleaning will also be done based on dental conditions As stated above adults may also be able to have their professional cleaning done at the initial visit IF gum health is normal.

Before Your First Visit-Patient Forms

Please download and fill out the following medical history, dental history, and patient forms as thoroughly as possible. The more accurate your information is, the better we can serve you. You can email or fax them ahead of your appointment or bring them with you to your appointment. Forms can also be texted or emailed to you via our patient portal system. Thank you!

Finance & Insurance

At Carolina Family Dentistry, we are happy to discuss fees and payment options with you before your treatment begins. We want your visit with us to be as smooth as possible, which is why we accept assignments from insurance plans and file the appropriate claims and paperwork for you!

Insurance

If you have insurance, our team would be happy to help you determine the coverage you have available. However, we do ask that you do your best to familiarize yourself with YOUR coverage and limitations as well as possible. We are happy to help you, file your claim, and handle insurance questions from our office on your behalf. However, our office only has limited access to your insurance policy information.

Dental Financing

We accept CareCredit. North America’s leading patient payment program, CareCredit is the credit card exclusively for healthcare services and can provide loans for up to 12 months at 0% interest. Not only will you have low monthly payments for any dental procedures not covered by insurance, but you are welcome to use your CareCredit card again and again at any follow-up appointments. We are pleased to offer the following conveniences:

Discounts for all patients who make full payment at the beginning of treatment
Discounts for senior citizen patients
Discounts for patients with reduced-fee plan insurance (PPOs, HMOs, etc.) Note: These are built-in to the insurance plan and have already been made. Ask our expert financial coordinator for details and for any other application specials and discounts.

My family and I started going to Dr.Banik for almost 29 years. They will make sure you get an appointment as soon aspossible. You’re also in good hands when you’re there.Oh yeah by the way for the last fifteen years, I’ve suffered with back problem, I’m anemic, I suffer with low blood and I’m always cold. When I come in for my appointment they always have a pillow and a blanket already for me in the room.Now how good is that to make me feel special.

So I just experienced my second visit with Dr. Banick. Instead of feeling like a dentist visit, it felt like I went to a magic show because what he did to my tooth that I have been looking at for a couple years, and it didn’t look pretty, looks amazing! I was blown away! And I can’t say enough about the staff, they are fantastic. I’m actually looking forward to my next visit. Thanks Dr Banik.

Always a pleasant experience. Dr Banik is fantastic as well as the entire staff [very professional and courteous) I had many dentists in my life and have very sensitive nerve endings. This is the only operation that has been able to address the issue and remedy it. I VERY highly recommend this dentistry practice.
